IMPLEMENTATION OF THE INTERNATIONAL DYSPHAGIA DIET STANDARDISATION INITIATIVE (IDDSI) IN ARGENTINA: RESULTS OF A NATIONAL SURVEY ON AWARENESS, BARRIERS, AND PERCEIVED BENEFITS

Abstract
INTRODUCTION: The International Dysphagia Diet Standardisation Initiative (IDDSI) provides a standardized framework for classifying foods and liquids for people with dysphagia. In Argentina, its implementation remains heterogeneous.
OBJECTIVE: To describe knowledge, level of implementation, barriers, challenges, and perceived benefits of the IDDSI Framework among speech-language pathologists and dietitians in Argentina.
METHODS: A cross-sectional, descriptive, observational study was conducted using a self-administered online survey. A total of 259 valid responses from professionals across 21 Argentine provinces were analyzed.
RESULTS: Participants included 135 speech-language pathologists (52.1%) and 124 dietitians (47.9%). Overall, 241 (93.1%) were familiar with IDDSI: 134 (51.7%) reported in-depth knowledge and 107 (41.3%) general knowledge. Implementation was complete in 71 (27.4%), in progress in 91 (35.1%), and had not been initiated in 97 (37.5%). Among the latter, the main barriers were lack of specific training, 69/97 (71.1%); absence of institutional protocols, 46/97 (47.4%); and limited resources, 45/97 (46.4%). Among respondents with complete or ongoing implementation (n=162), the main challenges were maintaining texture quality and safety, 116 (71.6%); improving communication across sectors, 112 (69.1%); and training new professionals, 98 (60.5%). The most frequently reported benefits were improved swallowing safety, 137 (84.6%); improved interdisciplinary communication, 126 (77.8%); and improved education and interpretation of recommendations, 119 (73.5%). A national implementation guideline was considered useful by 159/162 (98.1%).
CONCLUSION: Knowledge of IDDSI is high, although complete institutional implementation remains limited. The findings identify needs for training, standardization, and institutional support to facilitate its adoption.
